Accident/Incident Reporting

The University is committed to maintaining a safe campus and workplace. In an effort to do so, all accidents/incidents occurring on campus must be reported. Identifying causes and making necessary corrections when applicable will help minimize accidents and injuries. In the event of an accident or injury, employees, students, and/or visitors should document the incident using the electronic form found on the USA website located here:
